Senior Connections is an in-person case management program that connects older adults and dependent adults who have been negatively impacted and isolated by the COVID-19 pandemic to services that may enhance their general quality of life including social, recreational and informational activities.

Senior Connections can help:

• Reduce isolation
• Rejoin community
• Build circles of support
• Assess transportation needs
• Screen for technology access

Eligibility:

• Residents of Sacramento County
• Older adult (60 years old and older) -OR -
• Dependent adult (ages 18-59) who are unable to meet their own needs and are victims of abuse, neglect, or exploitation.
