Transition out of a nursing facility and into your home or to assisted living

In partnership with Anthem health plan and Sacramento nursing facilities, CalAIM Community Transitions Program provides case management community transition services to support people leaving the facility to live in a community-based setting.

Trained, gerontological case managers provide person-centered care to qualifying Anthem members who want to move out of the facility where they reside. AAA4 staff help them with searching for and securing housing and in-home care and establishing a safety net and contacts to retain housing.

We may also be able to connect residents to non-emergency, non-medical transportation to assist mobility, and identifying resources for environmental accessibility modifications.

• Have you been in a nursing facility for 60 or more days?
